Cosma Institute is promoting a more personalized approach to healthy aging, calling for a shift in aesthetic medicine away from conventional “anti-aging” treatments and toward strategies that account for how an individual’s entire face changes over time.
The approach focuses on understanding facial anatomy, maintaining harmony among facial features and helping patients preserve their recognizable appearance as they age, according to Cosma Institute founder and aesthetic physician Dr. James Co.
“Aging is not a disease we need to cure,” Co said. “It is a biological process. As physicians, our role is to understand what is changing beneath the surface, how those changes differ from person to person, and whether intervention is appropriate in the first place.”
Facial aging involves changes not only in the skin but also in bone, muscles, ligaments and facial fat. Because these structures can change at different rates, Co said aesthetic treatment should consider each patient’s anatomy, lifestyle, concerns and goals.
“Two patients can be the same age and come to me with completely different anatomy, lifestyles, concerns, and goals,” he said. “Why would we expect their treatment plans to look exactly the same?”
Cosma Institute applies this philosophy through consultation-led, whole-face assessments rather than automatically treating individual signs of aging. The approach places patient evaluation and individualized treatment planning at the center of aesthetic care.
Co also pointed to the growing visibility of aesthetic procedures on social media, saying it reinforces the need for professional assessment rather than treatment decisions based primarily on trends.
“You cannot freeze one version of your face forever,” Co said. “But you can understand how the face is changing and make decisions that respect its natural proportions and your identity.”
For Cosma Institute, the objective is not necessarily to make patients look younger. Instead, its healthy-aging philosophy aims to help patients navigate facial changes while maintaining natural proportions and remaining recognizably themselves.
